Q: What is the science behind CoolSculpting?
The science behind the Coolsculpting procedure is called Cryolipolysis, which was developed by world-renowned dermatologists at Massachusetts General Hospital in Boston, an affiliate of Harvard Medical School. Their research showed that fat cells are naturally more vulnerable to the effects of cold than other surrounding tissue and structures and that fat cells exposed to precisely controlled cooling can be safely eliminated without harming the skin. This discovery led to the development of the Coolsculpting procedure.
Q: How does CoolSculpting work?
During the procedure, a non-invasive applicator is placed on the patient, and the tissue being treated is brought into contact with cooling panels on the applicator. The Coolsculpting system delivers precisely controlled cooling to target and eliminate fat cells in specific body areas, such as the abdomen, love handles and the back. The fat is cooled over a period up to 1 hourper application site while the patient is sitting comfortably in the office.
Q: Are there any side effects and will patients experience frost bite from the cooling?
The Coolsculpting procedure is totally non-invasive and we have not observed any long-term or significant side effects. There may be some redness in the treated area which can last fromfew minutes to a few hours. Some localized bruising may occur, which typically clears in a few minutes to a few hours. Numbness and mild cramping can occur up to 2 weeks following the treatment. The procedure is designed to deliver precise and controlled cooling to the fat cells below the skin layer without harming the skin.

Clinical studies show that, for properly selected patients, the Coolsculpting procedure is a safe and effective way to reduce the fat layer, without the pain, risk, and recovery time of invasive procedures. The result is a noticeable, natural-looking fat reduction in the treated areas 2 to 4 months after a single Coolsculpting procedure. Better results can be obtained with a second procedure performed 2 months after the first. A multi-center clinical study in the U.S. in which Dr. Chapas and Dr. MacGregor were clinical investigators enrolled hundreds of patients and found that 84% of patients showed noticeable improvement after one procedure over a four month period. Ultrasound images indicated an average fat layer reduction of 22.4%. There were no significant changes to lipid blood levels or liver function during the post procedure period.

Q: Does it hurt? What does it feel like?
Our patients find the procedure to be gentle and easy to tolerate. Anesthesia and pain medication are not needed so patients can read a book, listen to music, work on their laptop or just relax during the procedure. When the applicator is placed on the skin, patients feel a pulling from the vacuum pressure that brings the skin into contact with the cooling panels. When the procedure begins, patients feel a cooling sensation that goes away after a few minutes as the cooling produces an anesthetic effect. After the procedure, there is no downtime and patients can resume their normal activities the same day, including work and exercise.

Union Square Laser Dermatology was one of the first centers in the U.S. to offer this treatment, starting in late June 2015. How does it work?
Kybella is a synthetic formulation of deoxycholic acid, which is a substance naturally synthesized by our body and used to break down and absorb dietary fat. Kybella has been under development for more than 9 years, and has been the subject of over 20 clinical studies involving more than 2600 patients. When properly injected in the chin area, it has been shown to dissolve submental fat (which causes double chin) and result in significant cosmetic improvement by creating a more streamline contour along the jawline.
What causes submental fat (double chin)? Who is affected?
The major factors seem to be genetics, natural aging or weight gain. Double chin can cause an older and heavier look and according to a 2014 survey by the American Society for Dermatologic Surgery approximately 70% of consumers report being bothered by submental fullness.
What other options exist to reduce submental fat?
The only effective options to treat submental fat before Kybella were surgery or liposuction.
When is Union Square Laser Dermatology going to offer Kybella?
Kybella was approved by the FDA in May 2015. Dr. Chapas is participating in Kybella’s training program to train physicians nationwide during the summer of 2015 (starting with our own physicians at USLD). Are the results permanent?
This is a recent treatment, so we don’t know for sure, but so far results appear to be permanent if there is no change in lifestyle factors.
What if there is subsequent weight gain?
Patients in the clinical studies who experienced weight gain after their treatment did not see return of their submental fulness. However we will have a better understanding about the impact of subsequent weight gain in the future, when data becomes available from a broader set of patients.
How many treatments does it take?
Kybella is administered by injections into the fat under the chin through a series of treatments spaced no less than one month apart. The treatment plan is tailored to the desired outcome by the treating physician. The total number of treatments varies–typically in the range between 2 and 5. We expect that most patients will receive two treatments spaced 1-2 months apart, but your physician will determine the best regime during your consultation based on your physiology and goals.
How quickly do the results appear?
Typically results appear within weeks to months, and keep improving as additional treatments are administered.
What is the evidence it works?
Kybella’s FDA approval was based on two clinical trials involving more than a thousand patients with moderate to severe submental fat, who received injections of either Kybella or a placebo. Patients that received Kybella saw a noticeable and cosmetically significant reduction of submental fat compared to patients that received the placebo.
Is it safe?
Side effects were comparable to other cosmetic injections such as dermal fillers, leading the FDA Medical Advisory Committee to unanimously recommend approval 17 to 0 after reviewing the data. In very rare cases, injection outside fat deposits could damage skin tissue, or injection too close to a nerve could cause injury that affects the appearance of the mouth. While side effects resolved on their own within 1-2 months of treatment, to minimize risks it is recommended that Kybella should be injected by dermatologists and plastic surgeons trained on its use.
What are counter-indications?
Kybella may not be appropriate for everyone. Patients that have had previous plastic surgery procedures on the neck, chin and jawline will need to be evaluated on an individual basis. Those who have difficulty swallowing, bleeding problems or medical conditions that might affect the face and neck may not be good candidates for Kybella.
Cellulite is present in 90% of women – of ALL shapes and sizes! Cellulite is genetic and not tied to weight. QWO is a non-invasive injectable procedure that treats moderate to severe cellulite in the buttocks of women. We’ve also been treating the upper thighs with similar success! By injecting directly into cellulite, Qwo may help to breakdown or release the collagen-rich bands that pull on the skin surface and contribute to “dimpling.”
UnionDerm served as clinical investigators in the research that led to the FDA-approval of this injectable in 2019. We saw significant improvement in patients’ cellulite dimples and the most long-lasting results (several years) in the entire cellulite treatment category. VelaShape 3 is a new non-invasive treatment that uses a combination of technologies to contour your body and reduce the appearance of cellulite. Vacuum technology manipulates your skin while infrared light and bi-polar radio frequency energies gently heat fat cells and surrounding tissue.
After 3-6 treatments, patients typically notice a reduction in both circumference and in the appearance of cellulite in the treated areas. The surface of the skin will feel smoother and firmer, too. Isolaz is a safe and effective acne treatment. It purifies pores deep down to attack acne causing bacteria at the source. Isolaz treatment is great for all acne, except cystic lesions, and can treat all skin types, light and dark. During your consultation, the physicians will evaluate your acne and determine which acne treatment is right for you.
The Isolaz technology was first introduced in 2005. Dr. Anne Chapas and Dr. Jennifer Chwalek have been using the device since it was first introduced. Isolaz technology combines vacuum with a broadband light to deliver effective acne treatments. The vacuum helps loosen and extract dirt, blackheads and excess oil from deep within your pores. Then the light helps destroy acne causing bacteria, as well as reducing facial redness.
After cleansing and steaming the skin, the laser treatment on the face takes 15 to 20 minutes, whereas treatments on the back and chest may take a little longer. Typically, two to six treatments are needed. However, the exact number of treatments varies from patient to patient and will be determined by your physician.
Typically, patients can see a visible improvement in the redness or severity of their acne after the first treatment.
